Updated Advance Care Directive for South Australia

Form updates reflect the new guidelines issued by the SA government.

Aligning with the Latest Government Updates


The South Australian government recently released a new version of the Advance Care Directive form. This update comes as a result of feedback from the 2019 legislative review, and input from the public, key stakeholders, and the health sector. 

As with the previous form, optional sections are required to be 'crossed out' if they are not selected or answered, to ensure clarity and precision and to make the document more straightforward for healthcare providers and other relevant parties.

Smarter Drafter: Simplifying Compliance

Our updated Advance Care Directive form within Smarter Drafter now incorporates these new requirements. With Smarter Drafter, all necessary formatting is handled automatically, ensuring that the final document is complete and compliant without the need for further editing.

Enhanced Guidance and User Experience

In addition to updating the form itself, we've also revised the questionnaire and guidance notes within Smarter Drafter. These revisions are designed to guide our users through each part of the form, ensuring they provide all necessary information in accordance with the latest government updates.
